When completing the form, please:

  • Provide all requested personal information.
  • Sign this form.
  • Clearly identify each item in your report you believe is inaccurate, incomplete or outdated. You may include a copy of your report with the items in question circled.
  • Clearly identify why you are disputing each individual item.
  • Identify if the item should be removed (such as expunged or sealed) or provide the correct information where appropriate.
  • You may attach copies of documents that you feel support your dispute. Supporting documentation is not required in order to submit a dispute.
